This program partners with local health plans to deliver community-based, person-centered care management for individuals with complex medical and social needs, using an interdisciplinary approach.
Behavioral Health Services Team
This program, in partnership with Stanislaus County Behavioral Health and Recovery Services, provides no-cost outpatient mental health services—including assessment, treatment, support, and crisis support to children and youth (ages 0–21) who qualify for specialty care.
Intensive Community Support
This free program offers outpatient mental health services for children and youth ages 0–21, including therapy, case management, and crisis support. Services are provided by a team of professionals and are available to adopted and foster children. Referrals can come from schools, parents, or community partners.

Wellness Groups
Wellness Support Groups provide assistance to undocumented students in postsecondary education who need support in their wellbeing, finding a support network, and feeling a sense of belonging. Each group includes 10-12 participants in a closed virtual space with a trained mental health provider or registered clinicians for 6 weeks. There are five different groups:
